In the first instance, please telephone Elm Tree surgery on 01642 616663 where we will assist you. To register you will be asked to complete a new patient questionnaire (we will email this to you on receiving your GMS1 form a Patient Registration Form)
You will be unable to directly register from the website. You will need to print or email this and attach two forms of ID (1 photographic and one Address ID) to complete your registration. If your registration request is rejected you will be provided with a full written explanation.
Elm Tree Surgery is accepting new patients. In order to register with the practice you must live within the Practice boundary (for further details of this you can contact us on 01642 616663 and enquire at Reception if you are within the practice boundary area. Presently, we are registering from Thornaby, Ingleby Barwick and only a certain part of Stockton.
If you move outside the practice boundary you must inform the surgery and register with a local doctor as soon as possible.
