Stockton Service Navigation Project – Can help you improve your health and general well being

The Stockton Service Navigation (SSNP) can help with non-medical problems. It offers up to four one-to-one support appointments which can improve your health and well being by supporting you to access a range of activities. This is known as social prescribing and aims to help people increase their confidence, control, independence, reduce stress and isolation and help people live healthier lives.

Patients needing advice about termination of pregnancy may refer themselves directly to:
